Small AVM at bifurcation of the left ICA and adjacent to M1 segment of left MCA with feeding arteries arising from left MCA and also left ACADiagnostic (DX):
Arteriovenous malformation (AVM), IVH and SAH

History (HX):
Headache, vertigo and blurred vision

Findings:
Inflammatory sinus disease in frontal, ethmoid and maxillary sinuses with mucosal thickening and retained secretion and right antrocoanal polyp extending from maxillary antrum through widened secondary ostium into nasal cavity above inferior turbinateDiagnostic (DX):
Antrocoanal polyp in right maxillary sinus and inflammatory sinus disease in maxillary, ethmoid and frontal sinuses.

History (HX):
An aphan man coming to Iran recently with headache, convulsion altered mental status and nuchal rigidity

Findings:
Mild communicating hydrocephalus with suspicious signal change at basal cistern on FLAIR images. On post-contrast MRI which has done 3 days later leptomeningeal enhancement is seen. Communicating hydrocephalus  is increased.

Diagnostic (DX):
T.B meningitis

Discussion:
Polymerase chain reaction (PCR) CSF was positive for Tuberculosis

History (HX):
Hypertension, limb claudication

Female   35 yr

Findings:
Multifocal stenotic segment at descending thoracic and abdomina aorta most marked between the origin of celiac trunk and SMA. Occlusion of proximal portion of left subclavian artery, stenosis at proximal portion of left common carotid and midportion of right subclavian arteries

Diagnostic (DX):
Takayasu Arteritis

Discussion:
A primary arteries which involves the aorta and its major branches in addition to pulmonary artery

History (HX):
Epilepsy for 10 years

Male   33 yr

Findings:
Multiple small subependymal nodules lining the lateral wall of the lateral ventricles which show same signal intensity as cortex (GM)Diagnostic (DX):
Nodular subependymal heterotopia

Discussion:
Contrast enhanced MRI is recommended to R/O remote possibility of Tuberous Sclerosis

History (HX):
Palpable mass and pain during a few months

Male   39 yr

Findings:
Large well-marginated solid and cystic mass in lower abdomen and pelvis behind the urinary bladder and above the rectum suggestive of a retropritoneal mass.
The solid components of the mass shows marked enhancement on post-Gd images.

Diagnostic (DX):
Low grade spindle cell sarcoma in favour of schwannoma (proved)

History (HX):
Paresis of both upper limbs and right lower limb

Male   75yr

Findings:
Sagital and axial T2WI show symmetric, longitudinally ariented zones of high signal intensity involving the dorsal columns of the spinal cordDiagnostic (DX):
Subacute combined degeneration of the spinal cord (SDC of the spinal cord)

Discussion:
Intramedullary signal abnormalities can resolve after correction of the vitamin B12 deficiency

History (HX):
History of chicken pox 2 weeks ago presenting  with seizure and altered mental status and disorientation (now in ICU) Male   8 yr

Findings:
Brain MRI shows gyral swelling and hyperintensity on FLAIR images in right temporo-occipital lobes and medial left temporal lobe . Diffusion restriction is present at these regions and also in thalami.

Diagnostic (DX):
Herpes zoster encephalitis with multifocal
